Clinical blood transfusion is not a trivial matter. How many checks and tests the patient goes through from blood collection, blood matching, and blood transfusion. The patient's blood bag is punctured during blood transfusion, causing waste and contamination of blood products, and even nurse-patient disputes, causing great distress to nurses. What should I do if the blood bag is accidentally punctured by the nurse?
If the damage is small, generally the following situations can be taken:
1. Keep the needle of the blood transfusion set at the socket of the blood bag, do not remove it, so as not to contaminate the blood components in the blood bag.
2. To remove the blood bag, generally the socket end of the blood bag needs to be up. Use iodophor to disinfect the punctured part of the Blood bag for 2-3 times, the diameter of the disinfected part ≥ 3cm of the punctured part.
3. At this time, take out the blood transfusion set, and choose a new sterile blood transfusion set to insert into the blood bag socket according to the aseptic operation specification to avoid puncturing again. Use iodophor to disinfect the punctured site again according to the above disinfection method, so that after the disinfectant is completely dry, use a sterile indwelling needle to paste the punctured site, and paste it flat (if necessary, use sterile scissors to cut the sterile dressing) No bubbles. If the paste is not tight, it will be prone to bleeding.
If there are too many damages, the blood cannot continue to be fed, and the blood continues to bleed, then you will consider yourself unlucky and discard the blood products decisively. This treatment method is simple and effective, and can effectively avoid patient infection.
After the blood bag puncture incident, how can the operators improve their work?
Strengthen clinical training. The clinic needs to strengthen professional nursing knowledge training for nursing staff, recognize the characteristics of blood bag, and standardize operation to prevent accidental puncture.
Strengthen the sense of responsibility education so that the nursing staff can strengthen the sense of responsibility, treat them with caution, and deeply realize the seriousness of the problem.
Emergency plan drill.
